Terms of Service for Occupational Therapy Supervision/Mentorship with Lynsay McFarlane-Shaw, Occupational Therapist and Clinical Supervisor.

Service for OT Supervision /Mentorship customers:

• Supervision /Mentorship is agreed between Lynsay McFarlane-Shaw, OT supervisor, and supervisee/mentee/commissioner (organisation employing supervisee/mentee).
• Supervision /Mentorship will be conducted on Zoom / online sessions.
• Supervision /Mentorship will utilise a long arm model of supervision.
• Supervision /Mentorship meets the professional standards outlined by the RCOT and HCPC.
• Supervision /Mentorship frequency is agreed between both parties in advance.
• It is the supervisee's/Mentees' responsibility to book sessions in accordance with the package paid for in advance.
• Supervision /Mentorship pricing is decided by Lynsay McFarlane-Shaw, OT supervisor, and may be subject to change on a 30-day notice period.

Specifying payment terms:

• Payment must be made before the beginning of supervision/mentorship sessions.
• Payment is made via Stripe card payment or invoice–bank transfer.
• Payment is made monthly or payment in full for the agreed package before beginning supervision sessions.
• Under the Consumer Rights Act 2015, you are entitled to a 14-day cooling-off period, which must precede the start of supervision. You can waive your right to the 14-day cooling-off period if you would like supervision to commence earlier than 14 days.

Failure to pay or if one of the parties wants to stop the business relationship.

• If payment fails, it will be requested again.
• If payment fails, supervision /mentorship services can be withheld or withdrawn.
• Cancellation of supervision /mentorship services from either party is on a 30-day notice period, in writing.

Insurance, Professional Standards, and Ethics for Supervision:

• Each supervisee must be responsible for their own HCPC registration and DBS certification and have their own occupational therapy indemnity insurance, and Lynsay McFarlane-Shaw, OT supervisor, holds no responsibility or liability for the supervisee’s actions under legal and insurance purposes.
• The OT supervisor will follow professional guidelines set out by the Royal College of Occupational Therapy Code of Ethics and Professional Conduct and the Health Care Professions Council (HCPC) standards of conduct, performance, and ethics.
• The OT supervisor will hold a current registration with the HCPC: OT24756.
• The OT supervisor will have adequate professional indemnity insurance.
• The OT supervisor will have a current DBS certificate, Number 001543270680, which is renewed annually.
• The OT supervisor is registered with the Information Commissioner’s Office (ICO) for data protection; the registration number is ZA130347.
• The OT supervisor/mentor is compliant with the General Data Protection Regulations. See Privacy Policy.

Insurance, Professional Standards, and Ethics for Mentees:

• Each mentee must be responsible for their own professional registration/certification in relation to their country/region of practice; the mentee will have their own indemnity insurance, and Lynsay McFarlane-Shaw OT supervisor/mentor holds no responsibility or liability for the mentee’s actions under legal and insurance purposes.
• The OT supervisor/mentor will follow professional guidelines set out by the Royal College of Occupational Therapy Code of Ethics and Professional Conduct and the Health Care Professions Council (HCPC) standards of conduct, performance, and ethics.
• The OT supervisor/mentor will hold a current registration with the HCPC: OT24756.
